If you are an uninsured or a self-pay patient, you have the right to request a good faith estimate of expected charges prior to receiving services.
Effective January 1, 2023, we will no longer participate with Healthfirst. This includes all Healthfirst Plans. Current Healthfirst patients who wish to continue to receive services should switch to another health plan. **In some circumstances—for example, if you are being treated for cancer or pregnant— you can continue to see your Crystal Run provider under Healthfirst, after December 31, 2022, for a limited time.
